Health Commons Solutions Lab is a publicly funded, not-for-profit innovation lab that challenges leaders in health and social systems to think differently about their work and the people that they serve.

Founded in 2017 and housed within Sinai Health with support from the Ontario Ministry of Health and the Toronto Central LHIN (now Ontario Health), we work with a range of partners to co-design unique solutions that are founded on communities’ own knowledge and expertise, challenge established norms, and lead to lasting change. 

As a “Commons”, we believe that new ideas and solutions need to be shared. We’re committed to documenting our learning journey and building a free repository of tools that are available to anyone who wants to use them. Our tools are intended to be copied, “hacked” and improved upon by anyone who’s interested. 

We work directly with community members because we understand that people’s stories are the key to unlocking strategies to improve health, and that focusing on people most at risk will yield the greatest gains. We think people’s own narratives, combined with available data, are the starting point for co-creating new and innovative solutions that help improve outcomes for those most impacted by the social determinants of health.

Our work is guided by a Theory of Change that describes who we are, what we do, and the impact we want to make.

The Health Commons theory of change is rooted in a mission to achieve health and dignity for all, through equity-based and population health models that are rooted in community.
