Building the Foundations of Population Health
Why It Matters If we care about the outcomes of our healthcare system, then the factors that shape them should be of great interest to us. We know that economic and social opportunity, environment, infrastructure, discrimination and education have a profound impact on shaping the health of whole populations. Understanding these patterns as the root cause of inequities is the first step to closing disparities in health.
What We Do Population health management provides the practical tools needed to redesign the healthcare system to make it more effective, proactive and rooted in equity. Health Commons is working with the Ministry of Health to develop practical use cases and tools to build capacity in population health.
